Diabetes Education & Support

Whether you have diabetes or are at risk for developing it, we can help you stay healthy.

Managing your diabetes is a lifelong commitment. Our diabetes education and self-management programs, recognized and certified by the American Diabetes Association, help you learn the skills you need to manage your diabetes, reduce symptoms and avoid potential long-term effects. Our diabetes education programs are available to you provided with a physician’s or nurse practitioner’s referral.

Taught by certified diabetes educators, our programs cover a wide range of topics, including healthy nutrition, weight management, medication management, stress reduction, physical activity, blood sugar monitoring and prevention of short- and long-term complications.

Free Prediabetes Education Program

If you are at risk for developing diabetes, this monthly program can help. Focused on prevention, the curriculum teaches you how developing healthier habits – including weight loss, proper nutrition, physical activity and stress management – can help you avoid getting diabetes.

You may be at risk for diabetes if you have:

  • High BMI (body mass index over 25)
  • Family history of diabetes
  • History of gestational diabetes or having had a baby that weighed more than 9 pounds
  • Inactive lifestyle
  • Polycystic ovary disease
  • “Prediabetes” with fasting blood glucose level greater than 100 but less than 126

Adult Diabetes Self-Management Program

If you have diabetes, this three-step program, certified by the American Diabetes Association, will teach you how to properly manage your condition.

  • Step 1: Assessment. Meet with a registered dietitian and a diabetes nurse educator for a one-on-one personal assessment.
  • Step 2: Education. Participate in group or individual education sessions. Topics covered in these sessions include the seven focus areas: healthy eating, being active, monitoring, taking medications, problem solving, reducing risks and healthy coping.
  • Step 3: Management. Work with our instructor to develop a customized, long-term plan to manage your diabetes, including setting goals and scheduling the appropriate follow-up evaluations.

Gestational Diabetes Program

If you have been diagnosed with gestational diabetes, we will help you manage your condition to protect your health and the health of your baby. We offer one-on-one meetings with a nurse educator and a dietitian who will review techniques for monitoring and maintaining your blood sugar throughout your pregnancy. We also provide (free of charge) a glucose meter and initial testing supplies for use during the program.

Pediatric Diabetes Program

Children and adolescents with Type 1 diabetes face special challenges. We work closely with young patients, their families and their primary care physicians to teach self-care and management techniques and to establish healthy, lifelong habits for managing diabetes.

Preoperative Nutritional Counseling

If you have diabetes and are planning to have bariatric surgery, our nutritionists can provide you with the preoperative nutritional counseling required by some insurance plans.

Technology: Glucose Meters, Insulin Pumps, Continuous Sensors

We make it easier for you to manage your diabetes by providing education and instruction regarding glucose meters, insulin pump management and continuous glucose sensors.

Educational Workshops and Support Groups

You may benefit from a variety of patient and family support groups, including:

  • Healthier Living Workshops. Learn techniques to improve your health and feel better while managing your chronic condition.
  • Insulin Pump Support Group. A monthly discussion of equipment, new technology, and other pump-related issues and concerns.
