Whether you have diabetes or are at risk for developing it, we can help you stay healthy.
Managing your diabetes is a lifelong commitment. Our diabetes education and self-management programs, recognized and certified by the American Diabetes Association, help you learn the skills you need to manage your diabetes, reduce symptoms and avoid potential long-term effects. Our diabetes education programs are available to you provided with a physician’s or nurse practitioner’s referral.
Taught by certified diabetes educators, our programs cover a wide range of topics, including healthy nutrition, weight management, medication management, stress reduction, physical activity, blood sugar monitoring and prevention of short- and long-term complications.
If you are at risk for developing diabetes, this monthly program can help. Focused on prevention, the curriculum teaches you how developing healthier habits – including weight loss, proper nutrition, physical activity and stress management – can help you avoid getting diabetes.

If you have diabetes, this three-step program, certified by the American Diabetes Association, will teach you how to properly manage your condition.
Learn about the AADE7 Self-Care Behaviors™
If you have been diagnosed with gestational diabetes, we will help you manage your condition to protect your health and the health of your baby. We offer one-on-one meetings with a nurse educator and a dietitian who will review techniques for monitoring and maintaining your blood sugar throughout your pregnancy. We also provide (free of charge) a glucose meter and initial testing supplies for use during the program.
Children and adolescents with Type 1 diabetes face special challenges. We work closely with young patients, their families and their primary care physicians to teach self-care and management techniques and to establish healthy, lifelong habits for managing diabetes.
If you have diabetes and are planning to have bariatric surgery, our nutritionists can provide you with the preoperative nutritional counseling required by some insurance plans.
We make it easier for you to manage your diabetes by providing education and instruction regarding glucose meters, insulin pump management and continuous glucose sensors.
