Provider of the Year Nomination Form

2023 Providers of the Year:

The Provider of the Year Award was created in memory of Dr. Emelie Munson Perkins, a multi-talented individual who served the Rutland Community for many years. The purpose of these awards is to recognize a:

  • Rutland Regional Medical Center Physician
  • Primary Care Physician
  • Rutland Regional Medical Center Nurse Practitioner / Physician Assistant
  • Primary Care Nurse Practitioner / Physician Assistant

whose contributions to our community, staff and patients represent the highest ideals in healthcare.

Award recipients will be honored at the Medical Staff Excellence Celebration Ceremony on Friday, March 28, 2025 at the Southside Steakhouse.

You are welcome to nominate more than one provider from each group. Thank you for helping us to recognize the contributions our providers make to our patients, our hospital, and our community.

Medical Staff Services operates as a liaison between the Medical Staff, Hospital Administration and Community Providers. The office is located within Administration on the first floor of Rutland Regional Medical Center.
