If you or a loved one are suffering from a behavioral or mental health condition, Rutland Regional Behavioral Health is here for you. Our team of caring, compassionate behavioral health specialists accept referrals from physicians and mental health professionals.

Referring physicians must complete a referral form and fax it to 802.747.0129. Physicians who submit referrals on weekdays before 4pm will be contacted by the end of the next business day. Rutland Regional Behavioral Health is closed on weekends.

Insurance Coverage

Our support staff are familiar with the benefits provided by many insurance plans and can help you with your insurance coverage questions; however, you may want to check with your insurance company about copayments and referral requirements for your specific plan.

What to Bring

When you schedule your first appointment, our skilled support staff will provide you with all the information you need to help you make the most of your visit.

When you arrive for your first visit, you will be asked to complete patient intake forms, including release-of-information forms that allow Rutland Regional Behavioral Health to coordinate your care and share information with your primary care physician or specialty physicians. To make sure your visit goes smoothly, we recommend bringing the following items:

  • Insurance card
  • Photo ID
  • Copayment, if required by your insurance
  • A list of treatments and medical/mental health history
  • A complete list of all medications that you are taking (including over-the-counter medications and supplements)
  • Notes from your last few physician visits, if applicable
